Making the Links: Transforming Long-term Conditions
Funded by the Department of Health Innovation & Excellence Fund, Making the Links is a three year project which aims to show the impact the voluntary sector can have
in supporting older people to self-manage their long term condition.
The project involves three local Age UKs (Leeds, Wakefield and Sheffield) working with GP practices in three local clinical commissioning groups. The practices will use an agreed risk assessment methodology to assess older patients at risk of hospital admission due to their long-term condition. Health professionals will then "socially
prescribe" Age UK support for a number of those older people to enable
them to self-manage their conditions more effectively. The local Age UKs will assess
and support patients to manage their conditions.
The objective is either to prevent an increase or to decrease the risk of the older person needing hospitalization because of their condition.
We are currently working with evaluators and information governance specialists to access the information we need to assess the project.
